Author Paperwork and Official Documents


New authors are sent a Welcome Email when they first sign up, containing all of the documents listed below. In case you have misplaced your welcome email, these are links to download them if you ever need to submit a new copy or want to do a whole new book.


Instructions on How To Download, Save and Submit the Forms
Click on the desired form link to download it to your computer. Often it will download to a special folder such as Downloads, Documents, or a pre-designated place such as the Desktop. Copy and/or store the file in a place that you know where to find it. They are all fillable, which means that they can be filled out entirely on your computer without the need to print them out and scan them. They can still be printed out for your records if you prefer a physical paper copy.

1. Download and open the form on your computer.
2. Fill in each line, or at least all of the required form fields.
3. Only then, most importantly, do a File>Save As with a modified name. (For example "SignedPrintBookPublishingAgreement.pdf".) This way, it will be identifiable to us, and if needed, the old blank form will be preserved and can be used again. File renaming is a must for Publishing Agreement contracts. These should ideally have the book title within the file name, along with Publishing Agreement.
4. If you already have an account with us, please log into your account and upload by clicking on the little yellow files folder icon within your Personal Profile.
5. If you do not have an account with us yet, please register for one. If you have any trouble logging in or creating an account, contact us at clientsupport@tovimpress.com.


Contracts
These are legal Publishing Agreements that contract us to publish your book. They are needed preferably as soon as you decide to publish with us, but they must be signed prior to your book's final submission for printing and distribution. We cannot publish your book without them. They are ISBN-specific, so if you want to do a print book in hardcover and a second print version in paperback, you will need to fill out two, one for each format. In the case of two print books with the same title, they can be Saved As with their proper ISBN. E-Books have their own separate Agreement as well. Below are the links to download your specific Publishing Contract. Choose which one applies to your book.

IRS Forms for Royalty Reporting
These forms are needed in order for you to receive royalties. We cannot issue you royalties without it. There are different forms depending on where you live. Since these can contain sensitive information, you must upload them directly to your account as stated above. This will keep all information secure and private. Emailing them is not recommended.

IRS Form W-8 for Non-US Citizens (Zipped file containing form and instructions. Needed once every three years.)


Other Types of Contracts
If you have taken a photo of anyone, whether it is an adult, a child or a pet, you must get the subject's permission before adding it to your book. The adult is easy, but the child or pet requires the guardian's permission. Below is a link where you can purchase these permission forms for download from our Picky Press Store. It is a package of forms for these people to sign. A Model Release Form applies to adults and children, and a Pet Photo Consent Form for any recognizable pet owned by a human being. Alternatives are to get permission from the person, guardian or owner in writing, or blurring or distorting a person's face beyond recognition. The whole key is recognizability.
